Educational Institutions' Letter Of Agreement: Click To Open

Our legal department determined that we do not need to have legal contracts with schools for students to participate in education experiences in our facilities. We can continue to work with students and schools through a letter of agreement. Please have the school President, Provost, Chancellor, or appropriate designee sign the Letter of Agreement. Then return the Letter with a copy of your certificate of insurance to the address or email indicated at the bottom of the letter.

Faculty- Require all checklist items initially and items 1 through 7 every year thereafter by January 15th.

Medical Students, Physician Assistant Students, Nurse Practitioner Students PharmD Students and Student Nurse Anesthetists-Working or located in inpatient or outpatient areas.

Post Secondary - Clinical Students- Working with patients or family or located in inpatient or outpatient care areas.

Post Secondary - Non Clinical Students- Not working with patients or family and not located in inpatient or outpatient care areas.

Observation Student more than 16 hours- High School, College, Graduate Students, Preceptorship for observation.
